pls, add ID for Radeon 5570 Oc Version

today i switch from Sapphire Radeon 6450 to Gigabyte Radeon R5570 OC version with 2GB and card do not work.

i think ID is missing

device/type 0x3
device/vendor 0x1002
device/id 0x675f
